Advisement / Registration Form

This form should be used if the student cannot access self-service or has received email permission from the appropriate Program Director, Dean or Provost. Please use the upload section to attach picture id and any email(s)/document(s) that will support your request. Missing required information or documents may delay or deny this request.

Drop / Add Form

This form is used to process a drop, add or both if a student is unable to process through self-service. Please use the upload section to attach picture ID and any email(s)/document(s) that will support your request from the appropriate faculty member, Program Director, Dean or Provost. Our staff will review and process accordingly.

Late Withdrawal Request

A late withdrawal request may be submitted after the published last day to Withdraw if it is approved by your instructor. In the last three weeks of the fall or spring semester a late withdrawal request must be approved by the Provost and Vice President for Academic Affairs who is RCC’s Chief Academic Officer.

(Visiting / Non-Degree Seeking)

A visiting/non-degree student is not pursuing a degree/certificate at RCC. You are taking classes to transfer to another college or for personal enrichment (visiting students are not eligible for financial aid).

Transfer Credit

Permission to Attend Another Institution

Any registered student planning to take a course(s) outside of RCC and looking to receive RCC credits for said course(s) must complete the Permission to Attend Another Institution. Please allow enough time for your account and course(s) to be reviewed.

SUNY Cross Registration

Cross-registration occurs when one SUNY institution (the Host institution) provides instruction for a student enrolled in a degree or certificate program at another SUNY institution (the Home institution) during the same academic term.

Transfer Credit Appeal

This form is used when a course(s) has been denied, and the student would like to appeal the decision to the appropriate program director, dean, associate provost or provost. The appropriate syllabus must be attached and any other supporting documentation.

Transfer Credit Course Description/Syllabus Review

This form should be used if a student would like to have a course(s) from their prior college reviewed to be considered as an equivalent RCC course.

Transfer Credit Re-Evaluation Request

This form should be used when a student has changed or added another curriculum/degree program to their profile and would like to have their prior college transcript(s) reviewed for additional transfer credits.

Academic Forgiveness Application

The College’s Academic Forgiveness Policy is designed to help students recover from past academic problems. There are two options for academic forgiveness. For more information, please read the policy linked below. If you have any question or wish to apply speak with any advisor first.

Application for Graduation

To obtain a degree or certificate from Rockland Community College, students must apply for graduation. It is recommended that students apply for graduation at the beginning of their last semester. Contact your advisor for more information.

Change of Information

Request a name change, address change, phone number change, social security number update.

Curriculum Change Request

Use this form to change your program of study, concentration, catalog or to add an additional approved program of study, certificate or microcredential.

Duplicate Diploma Request

Request a duplicate copy of your diploma. All duplicate diploma requests must be made in writing and submitted with photo ID (drivers license, student ID, passport) to the Registration & Records Office.

Enrollment / Graduation Verification

This form is used to request a letter or to have a form completed that will verify a student’s enrollment or graduation status. Please make sure all student (and parent/guardian) sections are completed, signed, dated and picture ID is uploaded to your request.

Pass / Fail or Audit Request

This form is used when a student would like to take a course on a Pass/Fail or Audit basis. Professor’s permission is required for all requests. Students receiving financial aid must also have permission from the Financial Aid Office.

NYS 24-Credit High School Equivalency Review

Upon completion of the 24 credits required to earn the HSE Diploma students must complete the review application below and submit it to the Records & Registration office. Additional information will be provided after the review is completed.

Immunization

NYS Public Health Law 2165 requires that students registered for 6 or more credits provide proof of immunity against measles, mumps and rubella. In addition, NYS Public Health Law 2167 requires students either show proof of receiving the meningitis vaccine within the last 5 years or complete the meningitis response form declining the vaccine.
